Submitting Proof of Vaccination

As per the GNWT Employee Vaccination Policy, all GNWT employees are required to provide proof of vaccination for COVID-19 on or before November 30, 2021.

What is acceptable proof of vaccination?

Acceptable proof of vaccination documentation must be an official record from the public health office in the jurisdiction(s) where the employee received their vaccine dose(s). The document must include:

  • The issuing authority of the vaccination record;
  • The employee’s full name;
  • The name/brand of the vaccine(s) received; and
  • The date(s) of vaccination.

Employees who are Northwest Territories’ residents and who received their COVID-19 vaccinations in the Northwest Territories can demonstrate their vaccination status by providing their electronic Proof of Vaccination Credential or a copy of their COVID-19 immunization record from Public Health.

How do I submit my proof of vaccination?

The following steps guide you through the submission process. For more detailed instructions, refer to the Submitting Proof of Vaccination document.

  1. Log in to SAMHRIS
  2. Click on the “Employee Profile” tile
  3. Click the “Vaccinated Status” tab
  4. Click the “Add” button
  5. On the new page, enter the date of your last vaccination (second dose or booster). This information is listed on your Proof of Vaccination credential.
  6. Click the “Add Attachment” button and select your proof of vaccination documentation.
  7. Click “Upload”.
  8. Click “Done” once the upload has completed.
  9. You will be brought back to the Vaccinated Status page. Confirm that the date of your last vaccination has been entered and that the correct document has been attached.
  10. Click “Submit for Approval”
  11. Click “Submit”

Once you have submitted your documentation, the status will show as “Pending Approval” and you will receive an email verifying that your documents have been submitted. Once approved, your status in HRIS will be updated and both you and your supervisor will be notified by email.

What should I do if my proof of vaccination is denied?

Your Proof of Vaccination could be denied for the following reasons:

  • Proof of Vaccination was not attached;
  • The ‘Date of last vaccination’ that you have entered in HRIS does not match the information on the document that you uploaded; or
  • The information in the document doesn't clearly state that you meet the the definition of "fully-vaccinated" and/or is missing required information.

If your submission is denied, you will receive an email that states the reason why. Once you have corrected the issue, you can resubmit your documentation using the same process.
